State of Decay 2 is an open-world survival horror game that builds on the combination of third-person action, base-building, and the ongoing management of a community that existed in the first game by including four different maps, co-operative multiplayer, and a more detailed simulation of a zombie apocalypse in which each survivor has a name, a backstory, and a limited lifespan. The players lead a growing group of survivors, each of whom has individual characteristics, abilities, and personal idiosyncrasies—some are natural leaders, some are headstrong lone wolves, and others have trauma that causes them to act in unpredictable ways—allowing them to switch between these characters as they search through abandoned towns, eliminate the plague hearts, and recruit new allies from the wasteland.

The main element of the game is based on the acquisition of resources and the sustainability of the community: food, medicine, ammunition, fuel, and building materials are continuously used up by the base, and the only way to keep all the survivors alive is to go into the hazardous, zombie-filled world to obtain more. Each trip has significant consequences—survivors get tired, are injured, catch the blood plague, and can die permanently—while the world itself changes dynamically, with infestations spreading, hostile enclaves taking over territory, and supply caches running out over time, making laziness a sure way to die.

The permanent death feature has been brought back and now has an even stronger emotional effect: once a survivor has died, they are lost for good, and their departure has a lasting effect on the community—others may grieve, morale could drop, and the skills they had (for example, medicine or mechanics) disappear until another survivor is trained to take their place. Base management has been greatly expanded, allowing players to build larger and more customizable home sites which include special built-in facilities (such as a fire station with a water tower or a mall with an integral generator) and the possibility of constructing particular rooms—like infirmaries, workshops, gardens, watchtowers, and even a fighting gym for training combat skills—all of which use up resources and need survivors to be assigned to them, thus requiring a constant balance between expansion and sustainability. The Blood Plague mechanism includes a second infection system in which survivors bitten by plague zombies begin a slow, irreversible transformation unless they are treated with rare cure vials, adding a time-limited element to each combat situation.

This is the first instance in the series where co-operative multiplayer lets up to four players join a host’s game, allowing members of their own community to take part, share resources, and deal with plague hearts, infestations, and hostile enclaves—yet only the host’s world advances. Hence, each session involves a mutually beneficial but ultimately temporary partnership. The true aim is not a conventional victory, but rather to survive over several in-game days, ending with a final, desperate effort to eliminate all the plague hearts and get out of the area, with the future of your community and the legacy of every survivor—both living and dead—having a significant impact on each decision.
